Cannabis for Multiple Sclerosis: Spasticity, Pain and Quality of Life

Multiple sclerosis is one of the best-documented indications for medical cannabis. Nabiximols (Sativex®) is approved in Germany for moderate to severe spasticity.

## What is Multiple Sclerosis?

Multiple sclerosis (MS) is a chronic inflammatory disease of the central nervous system. The immune system mistakenly attacks the myelin sheath of nerve fibres, creating lesions that disrupt nerve conduction. MS affects around 2.8 million people worldwide, approximately 280,000 in Germany.

## Sativex® – the First Approved Cannabinoid MS Medication

Nabiximols (trade name Sativex®) is an oromucosal spray delivering THC and CBD in a 1:1 ratio. In Germany it has been approved since 2011 for moderate to severe spasticity in MS patients who have not responded adequately to other anti-spastics.

Clinical trials (CAMS 2003, MUSEC 2012, MOVE 2 2014) demonstrated significant reductions in spasticity scores. Around 40% of patients are good responders, defined as ≥20% improvement on a Numerical Rating Scale.

## Beyond Spasticity: Pain, Bladder and Sleep

Evidence also supports cannabinoid use for neuropathic pain, overactive bladder (with significant reductions in incontinence episodes) and sleep disturbance in MS patients.

## Summary

MS is one of the best-documented indications for medical cannabis. Sativex® is the only approved cannabinoid-based MS medication in Germany. MS patients should discuss the options with their neurologist.
